In the same chapter, Dr. Kureha talked about the Will of D and revealed that even the King of Pirates, Gol D. Roger, had the initial D in his name, adding to the mystery of the Will of D.

Although it has not been officially confirmed, knowing Oda, it is highly plausible that the manga creator hid the Gear 5 transformation during the Drum Island arc. Also, the mention of the D initial and Nika transformation hint in the same chapter adds to the mysterious element of the One Piece story, which is a specialty of Oda.
